PRP, hyaluronic acid, or cortisone for knee arthritis?

No knee injection is best for everyone. This evidence-based comparison explains where each option may fit, how quickly benefit may appear, what uncertainty remains, and which safety and cost questions matter before deciding.

Three options with different strengths and limitations.

These injections are intended to improve symptoms and function. None reliably regrows cartilage, reverses osteoarthritis, or prevents knee replacement. Exercise-based care, activity planning, and treatment of relevant health factors remain foundational.

    Corticosteroid, often called cortisone

    Best-known role: Faster short-term symptom relief, particularly when pain or inflammation is limiting rehabilitation or another important activity.

    Expected timing: Benefit can begin within days. Across guidelines and reviews, the clearest advantage is short-term, and relief commonly fades over time.

    Main tradeoffs: Temporary pain flare, transient blood glucose elevation, skin or fat changes near the injection, rare infection, and uncertainty about repeated use. A two-year trial of injections every three months found greater cartilage loss without better pain relief than saline; that repeated schedule is not the same as one selectively used injection.

    Coverage: Often covered, but plan benefits, authorization, and patient cost-sharing still vary.

    Hyaluronic acid, or viscosupplementation

    Best-known role: A non-steroid option for selected patients who remain symptomatic after foundational care and accept a meaningful chance of no benefit.

    Expected timing: When benefit occurs, it usually develops over several weeks and may last for several months. It is not an immediate pain reliever.

    Main tradeoffs: Average added benefit over saline is often modest, products and schedules differ, and guidelines disagree. Temporary pain or swelling can occur, with uncommon intense inflammatory reactions and rare infection.

    Coverage: Coverage is variable and may depend on the product, prior treatment, prior authorization, and plan-specific criteria.

    Platelet-rich plasma, or PRP

    Best-known role: An optional autologous treatment for selected patients, usually with mild-to-moderate knee osteoarthritis that remains symptomatic after appropriate conservative care.

    Expected timing: Temporary soreness is common. When benefit occurs, improvement is generally judged over weeks to months rather than immediately.

    Main tradeoffs: Preparations are not standardized, response is unpredictable, and rigorous trials include both positive and negative results. PRP has not proven reliable cartilage regeneration. Lightheadedness or bruising from the blood draw, pain flare, bleeding, and rare infection are possible.

    Product question: Ask about final platelet concentration, injected volume, total deliverable platelets, and cellular composition. The 2026 AAPM&R guidance identifies at least 10 billion total deliverable platelets as a consensus-based best-practice target, not a proven universal cutoff.

    Coverage: PRP for knee osteoarthritis is commonly self-pay. Confirm the full estimate and what it includes before scheduling.

There is no universal winner.

Corticosteroid generally offers the fastest expected relief. Several meta-analyses favor PRP over corticosteroid and hyaluronic acid at later follow-up, but differences in study quality, patient selection, PRP composition, comparison products, and outcome reporting limit certainty. Hyaluronic acid remains a selective option despite modest average benefit and conflicting guidelines.

The timeline changes the answer.

A 2024 systematic review of 35 randomized trials found corticosteroid outcomes similar to hyaluronic acid and PRP at short follow-up. PRP produced greater average improvement than corticosteroid at later follow-up, while the longer-term pain difference favoring hyaluronic acid over corticosteroid did not reach the review's threshold for clinical importance.

Early weeks

Guidelines support corticosteroid for short-term relief. In a large-trial network analysis, triamcinolone was most likely to exceed the prespecified benefit threshold at two and six weeks, although that estimate came from one trial.

Several months

Hyaluronic acid may help selected patients, but the average added benefit is often small and not consistent across high-quality analyses.

Six to twelve months

Several comparative meta-analyses favor PRP on average at six to twelve months. Most evidence and current guidance focus on mild-to-moderate disease, and certainty is limited by heterogeneous products and protocols.

A 2025 meta-analysis limited to 15 double-blind randomized trials found PRP produced greater 12-month improvement than hyaluronic acid in pain and total WOMAC scores, with differences meeting the study's clinical-importance threshold. That result does not identify the best PRP product or guarantee benefit for an individual.

Placebo-controlled PRP evidence is also mixed. A 2025 meta-analysis pooled 18 placebo-controlled trials, including RESTORE, and found average benefit. RESTORE itself found no advantage from three weekly leukocyte-poor PRP injections over saline for 12-month pain or medial tibial cartilage volume. Both findings belong in an honest discussion.

A network meta-analysis is not a head-to-head promise.

Many rankings combine direct trials with indirect comparisons. A treatment can rank first on average while the differences remain uncertain, clinically small, or dependent on trial quality. Product choice, disease severity, and the patient's goal still matter.

Start with the person and the goal, not the injection name.

Corticosteroid may fit when

Faster short-term relief is the priority, symptoms are limiting rehabilitation, and health factors such as diabetes, infection risk, and possible surgery timing have been reviewed.

Hyaluronic acid may fit when

A gradual non-steroid option is preferred, other appropriate care has not helped enough, coverage or self-pay cost is acceptable, and expectations account for modest average benefit.

PRP may fit when

Mild-to-moderate knee osteoarthritis remains symptomatic, the patient accepts delayed and uncertain benefit, rehabilitation will continue, and self-pay cost is understood.

Questions that can change the decision

  • What is the diagnosis? Arthritis pain can overlap with meniscus symptoms, tendon pain, referred pain, crystal arthritis, inflammatory disease, or another condition.
  • What is the goal? Define a meaningful activity such as stairs, walking distance, exercise, sleep, work, or sport.
  • How advanced is the arthritis? Injection response is generally less predictable in end-stage disease, when surgical consultation may be more useful.
  • What has already been tried? Review exercise dose, activity changes, topical or oral medicines, braces, prior injections, and response.
  • What health factors matter? Diabetes, immune suppression, bleeding risk, medicines, allergies, pregnancy, active cancer treatment, and infection can change timing or eligibility.
  • Could surgery be near? A recent joint injection may affect arthroplasty timing. Discuss any possible knee replacement before proceeding.

Pause before an elective injection

A hot, rapidly swollen knee, fever, current infection, unexplained large effusion, new inability to bear weight, recent significant trauma, or major diagnostic uncertainty should be evaluated before choosing an elective injection. Aspiration, imaging, laboratory testing, or a different treatment may be needed.

Review shared risks and product-specific risks.

Any knee injection can cause temporary pain or swelling, bruising or bleeding, lightheadedness, injury to nearby structures, infection, or no meaningful benefit. Serious complications are uncommon but possible.

Report all anticoagulants and antiplatelet medicines. Do not stop a prescribed medicine unless the prescribing and procedural clinicians coordinate a plan.

Corticosteroid

Discuss diabetes and glucose monitoring, immune suppression, skin changes, prior steroid exposure, and possible surgery. If you have diabetes, ask for an individualized monitoring plan. Glucose may rise during the first several days after injection. Repeat injections should not be placed on an automatic schedule without reassessment.

Hyaluronic acid

Report prior reactions and relevant allergies. Some products are avian-derived. A pronounced inflammatory flare can mimic infection and needs prompt assessment.

PRP

Review all medicines and supplements, blood counts or bleeding disorders when relevant, infection, cancer treatment, and tolerance of a blood draw. Never stop a prescribed medicine on your own.

Get urgent medical guidance after an injection when symptoms are escalating

Seek urgent same-day medical assessment for fever, chills, rapidly increasing redness, warmth, swelling or pain, drainage, or a new inability to bear weight. Call 911 for trouble breathing, facial or throat swelling, chest pain, fainting, or another medical emergency.

Coverage can be as important as the evidence.

Corticosteroid

Often covered, although the specific drug, imaging guidance, facility, deductible, and copay may affect the final patient cost.

Hyaluronic acid

Coverage may require prior authorization, documentation of prior treatment, a plan-preferred product, or a defined interval since a previous course.

PRP

Commonly self-pay for knee osteoarthritis. Ask for the full estimate, including consultation, blood processing, procedure, imaging guidance, and follow-up.

    Suggests corticosteroid for persistent knee osteoarthritis pain not adequately relieved by other interventions. It finds insufficient evidence to recommend for or against hyaluronic acid or PRP, and insufficient evidence to choose corticosteroid over hyaluronic acid or vice versa.

    AAPM&R, 2026

    Supports considering PRP for selected patients with mild-to-moderate knee osteoarthritis who remain symptomatic after standard conservative care. Its guidance emphasizes patient selection, product characterization, platelet dose, image guidance, informed consent, and rehabilitation.

    AAOS, 2021

    States that corticosteroid could provide short-term relief, does not recommend hyaluronic acid for routine use, and states that PRP may reduce pain and improve function while rating the PRP recommendation as limited.

    ACR/Arthritis Foundation, 2019

    Strongly recommends intra-articular glucocorticoid for knee osteoarthritis, conditionally recommends against hyaluronic acid, and strongly recommends against PRP because preparations and techniques were heterogeneous and not standardized. Its review predates several newer PRP trials and the 2026 AAPM&R guidance.

A guideline is not a personal treatment order.

Publication date, evidence thresholds, product standardization, cost, and the population each group serves can produce different recommendations. Shared decision-making should include the best current evidence and the patient's goals, risks, and preferences.

Bring the information that changes the decision.

  • Symptoms and goals. Note what the knee prevents you from doing and the result that would make treatment worthwhile.
  • Prior care. Bring rehabilitation details, medication trials, braces, previous injections, response, and duration of benefit.
  • Imaging. Bring prior knee radiographs, MRI reports, and images when they are outside UCLA Health.
  • Medicine list. Include prescription medicines, over-the-counter pain relievers, blood thinners, vitamins, and supplements.
  • Health context. Report diabetes, immune suppression, infection, cancer treatment, bleeding problems, allergies, pregnancy, and possible joint-replacement plans.

Which injection is best for knee osteoarthritis?

There is no single best injection for every person. Corticosteroid is usually considered for faster short-term relief, hyaluronic acid is a gradual non-steroid option with modest and variable average benefit, and PRP has a favorable longer-term signal in several meta-analyses but greater product variability, uncertainty, and out-of-pocket cost.

Which knee injection works fastest?

Corticosteroid generally has the fastest expected onset. Hyaluronic acid and PRP are usually judged over weeks rather than days. No injection is guaranteed to help.

Which injection lasts longest?

Several comparative reviews favor PRP at six to twelve months, while hyaluronic acid can also provide relief for several months in some patients. These are average study results, not a guaranteed duration. Corticosteroid is best supported for shorter-term relief.

Does PRP, hyaluronic acid, or cortisone regrow cartilage?

No. None has been shown to reliably regrow cartilage, reverse osteoarthritis, or prevent future knee replacement. The usual goal is temporary improvement in pain and function.

Is PRP covered by insurance?
