How to make an appointment.

  1. 01

Choose the reason for your visit

Describe the body area, how long symptoms have been present, and whether there was a recent injury.

  2. 02

    Confirm the location

Ask whether Westwood or West Hills is the best fit for the visit type and available appointment.

  3. 03

    Check records and authorization needs

Ask what imaging, referrals, insurance authorization, or outside records should arrive before the appointment.

Selected ultrasound-guided procedures may require a separate appointment, authorization, or specific location. A consultation does not guarantee a procedure on the same day.

Prepare for a more useful first visit.

  • Bring your appointment confirmation, photo ID, and insurance card.
  • Bring a current medication list, including anticoagulants and supplements.
  • Bring prior imaging reports and the actual images when available.
  • Bring relevant operative reports, therapy notes, and procedure history.
  • Think about the activities and decisions you most want the visit to address.

Insurance participation and out-of-pocket costs can change. Verify coverage directly with UCLA Health and your health plan rather than relying on a static list.

Scheduled, urgent, or emergency care?

  • Scheduled sports medicine visit: persistent or recurrent pain, overuse injury, osteoarthritis, tendon symptoms, imaging questions, or return-to-activity planning.
  • Urgent orthopedic evaluation: a new non-emergency bone, joint, muscle, or tendon injury may be appropriate for UCLA’s orthopedic walk-in services or another urgent clinic.
  • Emergency care: call 911 or go to an emergency department for severe trauma, visible deformity, an open fracture, uncontrolled bleeding, loss of consciousness, or a numb, cool, pale limb.
